Lens replacement surgery — also called Refractive Lens Exchange (RLE) or Clear Lens Extraction — is a procedure where the eye's natural crystalline lens is removed and replaced with an artificial intraocular lens (IOL). It's the same surgical technique used in cataract surgery, but applied to patients who don't yet have cataracts.

How Lens Replacement Surgery Works

The procedure takes about 15–20 minutes per eye and is performed under topical anesthesia (numbing drops). Here's what happens:

1. A tiny incision is made at the edge of the cornea — no stitches required.
2. Ultrasound energy (phacoemulsification) breaks up the natural lens into tiny fragments.
3. The fragments are gently removed through the small incision.
4. The IOL is inserted and unfolded into the capsular bag where the natural lens sat.
5. Recovery begins — most patients see significant improvement within 24–48 hours.

Who Is a Good Candidate?

Lens replacement surgery is ideal for:

• Patients over 45–50 with presbyopia (difficulty reading up close due to age)
• High prescriptions that exceed LASIK's treatment range (above –12D myopia or +6D hyperopia)
• Thin corneas that make LASIK unsafe
• Patients who want to prevent future cataracts — once the lens is replaced, cataracts cannot develop
• Dry eye patients who are poor candidates for corneal-based laser surgery

Types of IOLs Available

• Monofocal IOLs — clear vision at one distance (usually distance); patients may still need reading glasses
• Multifocal IOLs — simultaneous clear vision at multiple distances; reduces need for any glasses
• Extended Depth of Focus (EDOF) IOLs — elongated focal range for natural vision across distances
• Toric IOLs — correct astigmatism in addition to other refractive errors

Is Lens Replacement Better Than LASIK?

For younger patients with stable prescriptions within LASIK's range, LASIK remains the gold standard. But for patients over 50, or those with very high prescriptions, refractive lens exchange often delivers superior, longer-lasting results because it addresses the root cause of vision changes — the aging crystalline lens.
